How to fill CS Foundation Exam form

My cousin took registration on Dec 2019, She is living at remote location so do not know anything about examination and all. She is prepared now for exam but do not know how to fill the exam form. She googled for link but not find. ICSI site has not much information regarding this.   Pls. help
