History Sheet for new assessee is not required.

No attachment / document / annexure required with ITR V or even paper returns except for ITR 7 applicable to Charitable Trusts / Societies.

If you have filed the return without a digital signature then send a copy of ITR-V (without any attachments) to CPC Bangalore. If you have filed with a digital signature then no need to send the ITR-V to CPC Bangalore.
