ITS DEPENDS UPON UR PROOF WHAT U R GIVING WITH DIN FORM FOR UR IDENTITY AND ADDRESS PROOF.

 Agree with Ankur. If you provide IT proof the name has to be given this way only.

However if you have any other proof (viz., DL, Voter's ID, Ration Card) showing his correct first , middle and surname you may use that as ID proof so that his correct name viz.,  Rajubahi(First Name) G(Middle name)  Rathod (Surname) gets reflected in the DIN.

Telephone bill can be used as a residence proof only. The name should be reflected as appearing in ID proof viz., IT PAN car, Driving Licence, Votors ID, Ration Card, etc.

Hi Mr.Rathod

When you are giving Electricity Bill as a proof, It doesn't matter how you fill DIN Application and how you write your Name and your Father's.

In the Electricity proof that you have sent, your full name is given. so write the name as the bill only.

If you don't have any proof with your Surname, then by attaching an Affidavit, you can use you Father's name as your Surname.
