Help in filing Form 108

So, i just completed my articleship but during articleship i received stipend in two different accounts for first 15-16 months in one account and then in a separate account both under my name due to some error in my initial account.

Now which account do I need to enter in Form 108 and Do i need to scan and upload any documents along with Form 108?
