what rate of GST will apply in a invoice in which the material GST rate 12%, 5% and 18%. I have
charged GST @ 18% on total material value. Is it correct or GST should be applied on material GST rates. Please clarify.

How it is calculated in a fixed cartage amount
You have to charged as per the rate of material only.
