Forms in case of conversion of Pref shares to Equity Shares

On conversion of Preference shares to Equity Shares, Which all form need to file with ROC?? Please guide me on this. Also please state where it is written in law? Moreover if form 2 is to be filled for issue of equity shares, it should be filled as issue for cash consideration or otherwise? Please state the reason for same. Thanks
