Please find attached the form for applying as FCA. I have submitted this form in the forum in reply of one of the clubmates asking a query in expert module. However, there is no option for attaching a file. Thank you.

Sir, there is also Form No. 3 for entry in the register as Fellow given in the website of the ICAI and here you have submitted a self declaration form. Kindly explain the difference between these 2 forms.
