Form A ????? there is no such Form available or prescribed in GST.

Form A - export oriented unit firm they have issued this form

Form A is not under GST...
It's used in customs clearance activities for export (and import).

Form A is Intimation for procurement of supplies from the registered person by Export
Oriented Unit (EOU)
