Form 117 for mdp firms and audit queries

I want to start a multi disciplinary firm along with a PCS. However, form 117 is not accepting his membership number (I have selected CA as type of firm and partnership firm in constituition in the initial selection). Any ideas on how to resolve this error? 

I have also read that for accepting statutory audit, the firm should be a majority CA firm and there are two criterias for that:

1. Based on the number of partners and

2. Based on the profit sharing ratio of the firms.

Are both the critieria required to be fulfilled in my case or only a single criteria can be fulfilled for accepting stat audit.
