Form 112: Query

Hello friends

I am pursuing both CS and CA course but i had not submit Form 112 to iCAI.  I joined CS last year when i was doind articleship with other CA and currently i am registered with other CA. Now i want to submit Form 112 but there is a confusion that from which CA i have to signed Form 112, previously or current. There is mention in form that form can be submitted late with fine, it only tell about late submission but thare is no mention about authorisation/ attestation/ signature of CA.

Please help, its urgent, thanks in advance.
