form 102, 103, 109... and articleship

Respected Friends,

I did not get my Articles Registration Letter and ID Card till date but the officials are saying that I have been regtistered as an Articled Assistant.

I want to take transfer and is it possible to take transfer without the above?

If yes, then please post an extract of FORM 109, so that I can fill it.

I have lost copy of form 109 which I had submitted to institute ..and I don't he copy with me nor my current principal ...now my articleship is almost completing I require form 109 details for leave taken and hrs worked ...how to apply for lost form 109 from institute...plz reply as soon as possible
