Fillip form

Hi,

I am trying to incorporate a LLP in which all 3 partners wants to become designated partners. I am applying 2 DIN in the fillip form. I have few queries.

1 Which form should I submit to apply for 3rd DIN?

2.Should I wait to get the incorporation certificate to submit the 3rd DIN or I can submit it simultaneously with FILLIP form. 

3. In the Subcriber's sheet and consent form to be attached in FILLIP form, should all partners mention their status as designated partners or only 2 partners can be mentioned as designated in the subcriber's sheet.
