Kindly guide me, how to file the FATCA CSR Declaration form for my client who is having the USD account with CITI Bank, when he was in US.

It would be advisable to ask you client to fill the Fatca details online over Bank's site. ( Login may be required).
Bank has prescribed forms, which is required to be signed in person.
