Exemption of CPE Hours

The exemption from complying the CPE hours requirements on the grounds of Pregnancy is granted for one Calendar year. The scanned copy of the following documents are required for claiming exemption :-

1. Birth certificate of the child as issued by the Govt. authority, and
2. A certificate from the gynaecologist or any other supporting document (eg. Discharge card) issued from the hospital in this regard. 

What is the procedure for claiming above pregnancy exemption ? Where to upload these 2 documents ?
