Tell your problem to icai by writing them through e-saahitiya.. N take necessary steps

Or tomorrow immediately call Delhi icai..
And also contact at the regional office of your city...

don't worry. your exemption is safe even if you not mention it in exam form

No need to worry. Exemption secured is valid for next 3 attempts, it will also be mentioned in your marksheet of may'16 attempt. Not mentioning it in exam form will not disentitle you from claiming exemption. Many of my friends did this mistake earlier but this didn't impact their claim on exemption.
Just 1 month before exams icai releases record of valid exemptions online, you can check yours by entering your registration no.
