Exam form query

Dear Sir, While registering for the Examination Form of IPCC Nov. 2016 , I entered the following details as per my registration with ICAI but my name was not shown on the site while registering for New account registration. Kindly advice me if I should go for provisional registration and in that please give the details of necessary documents required.
