The photo and sign are uploaded from previous record in exam form. Do we need to compulsory remove colour print out or black n white would do?

No such compulsion of taking colour printout. It's up to you. Your image will be visible in B&W printout too...
No such requirement. Just send it already. Between I saw that barcode was not drawn on the form as it was last time.
Just the control No. i.e.Barcode Number is there.
