I had a doubt w.r.t to Table A3 of Schedule FA for ITR2 AY20-21. The table has the following columns:

Say I had acquired shares via ESPP for Rs.10,000 in 01-01-2017 and have not sold them yet,
1. Can we enter Rs.10,000 for Column 8, 9, 10 ?
2. what should be entered in Column 11 (Total gross amount paid/credited with respect to the holding during the period )? is it Rs.10000 ?
3. If I haven't sold it yet I hope Column 12 will be Rs.0
4. Will "Nature of Entity" be ESPP?
Kindly share your inputs, I couldn't find any other online resources explaining it and lot of others have this doubt.
