Form 4 is for making changes in details like residential address , change in passport number voters card number or other details if you are filing it for address change then first get the details of changed address  on the passport or ration card and attach  the attested copy of ration card, passport through CA ,CS, ICWA or through gazzated offficer  and for any other changes get attested copy of original source document like changed voters identity card or changed pan card etc.and upload it,

If you are filing it for forign director then get it notarised from his home country or MD or CS of that particular Company .
