Following Documents are required to be filled.
1) Last 3 half yearly Return
2) All service Tax Liabilities should be paid.
3) Original ST-2
4) Authority Letter
5) Covering Page
also include the service tax return (ST-3) as on date i.e.date of surrender of Service Tax Return.
