Details in Form No - 108.

Can Any one tell me the details for submission of Form No

108..............

The Situation is that - 

I have competed The Training Duration of 3.5 years,,,,,,,,,,,,,

but i have took the training under two of the principals ......

5 month under 1st Principal

And 3 year and 1 month under Subsequent principal.

Can any one tell me that in Form 108 ,

Is it required to fill The Details of both the training ????????

or the Details of only subsequent training...????????

And in the Page No 4 Of the Form 108,

Where the Allocation of Weeks have to be Given
There are Only Collumn of 3 years,
But my Training Is More than three years........

So How can i Allocate Time In This Table.......
