I guess there is no such In-Principle form for delisting of shares from all exchanges.

You have to make an application to get In-Principle approval for delisting.

Also this website says the following  on delisting

"The Stock Exchange has not yet issued any forms under SEBI (Delisitng of Equity Shares) Regulations 2009"
