Condonation of delay in filing chg-4

I have to filed form CHG-4 for satisfaction of charge as i got NOC letter regarding release of charge from the bank on 12.11.2014., that means the form CHG-4 is to be filed within 30 days of the full payment (i.e. till 11.12.2014) as the form is filled along with digital signature and send it to the Chargeholder(i.e Banker) for the banker signature, but the banker has send me the form on 25.12.2014 (i.e after 13days of due date), now this delay is from the part of the Banker. 

My Question: 1) what will be the reason for delay as i had to mention it in the FORM CHG-4,

                    2)  Do i need to file FORM RD-2 along with CHG8 and Affidavit verifying the petition both as mandatory attachment in FORM RD-2, and at last after receiving any order such need to be filed in FORM INC-28. (Please Confirm)
