Company incorporation

I have to file spice - Inc 32 for the incorporation of of the company.
there is first 2 subscriber and both of don't have DIN. As per rule we have to attached identity proof and address proof of first subscriber if they don't have DIN.
But in form there's only one option to a attach proof of subscriber. so how should I attach the document for both subscriber.
Do I need to merge the proof of both subscriber to make it single?
