in condition icai indicate that no personal mobile number and communication address should be disclosed...and in prescribed format it is mandatory what should I do
Please note that the purpose of handwritten letter is that the ICAI should have a evidence in hand about your wish to get the papers verified or inspected. The objective is nowhere to divelge your details to the examiner. This could also be seen by the ICAI mentioning that papers to be checked / verified will be seen by the ticks you marked at website and not in your hand-written form
