certificate of service form -can any one help me

hi i hv appeared dis nov . for final examination so i will hv to submit certificate of service otherwise my result would not be declared . ICAI sent me that form but i have misplaced it and i hv searched out on ICAI site bt couldn`t find.

last date of submisson of that form is 30th nov. 2009

plz tell me frm where i can get copy of that form ??

CONTACT YOUR NEAR REGIONAL OFFICE OF ICAI.....OR CONTACT THE HEAD OFFICE AT NEW DELHI IMMEDIATELY.....

You can do fotocopy of that from from ur any freind because its all same form otherwise contact icai

I HAVE UPLOADED A FILE ON CACLUB IN WHICH YOU CAN FIND ANY CONTACT NUMBER OF ICAI IN WHOLE INDIA......../share_files/files_display.asp?files_id=20182
