Hi,
While filling my CA membership application form , I found that the last page of the form contains space for affixing our photo and our signature. Is that only what we need to fill and send for getting I-card, or do we have to fill up any other form for getting I-card?

There is a single page form for the id card

What you said is only for applying for the membership.
You have to send one more form for i card. which can be downloaded from icai site.
Menu is members- all forms- 2nd sl no form
