You select the format given for those who have completed articleship, that requires self attestation.

WHILE FILLING THE EXAM FORM WE NEED TO GIVE DETAILS OF LAST EXAM APPEARED
1) I APPLIED FOR MAY 2018 BUT DINT APPEAR FOR THE EXAM. SHOULD I GIVE ITS DETAILS?
2) ALSO I HAVE LOST MY MAY 2018 EXAM HALL TICKET AND DONT HAVE THE DETAILS LIKE ROLLNUMBER. WHAT SHOULD I DO

But principal' attestation is required or not... Or sign of any other member of icai is allow??
@ Divya For articleship completion attestation is mandatory only if your photograph and signature is appearing on the pdf.
If not appearing then you can get it attested by any member of ICAI.
