composition tax payer has to issue "BILL OF SUPPLY" quoting 'Composition Taxable Person Not eligible to collect tax on supplies'

There is no specific format for Invoice under GST, there is only specific information shall be printed on the invoices such as
Head of Invoice – Bill of Supply
Supplier Details – Name, Address, GSTIN,
Customer Details – Name, Address, GSTIN (if available) and other details
Billing to and Shift to address if Required
Details of goods – Descripttion, quantity, rate, value
Seal and Signature
