I want to apply for Certificate of Practice. What is the fees that I need to pay. What is the form I have to fill.

You will find Form no 6 Downloadable Version,
Take that & the procedure along with the Fees Payable is mentioned there on the Form itself. Fill the Form attach the necessary Docs mentioned therein & Submit the same along with your Cheque for Fees to the Regional Office & you will get the COP in due time
