Application for certified answer sheets - urgent

A prescribed format has been given for applying certified copies of Answer sheets by ICAI. For Verification application no format is given, we have to apply simply by writing in examinee's own hand writing in seperate paper and apply it. My doubt is, is it sufficient if we simply fill in the blanks in the format prescribed for applying Certified copies of Answer sheets in examinee's own hand writing or follow same procedure as prescribed for Verification application i.e., applying in a seperate paper by following the prescribed format? Application is for Certified Copies of Answer Sheets.
