Annexure 2 for service tax change of address

My client has presently shifted his business address to a new premises. i  filed an application for change in the address on the aces website and submitted the same with all the relevant document now they have asked me to file an ANNEXURE 2 after filling details online and then taking a print out and submitting the same to the department . i am unable to find the link to the same . please provide me  the direction urgently.

 

Did you get the Annexure 2 ? , if yes, where?

Annexure 2 is given for deletion of branches from Centralised registration(ST-2 certificate).
