Affidavit title

Respected experts, My PAN is lost hence I am in need of a duplicate PAN for which an affidavit is to be executed and submitted to the authorities.stating the place of Birth & date of birth. Here in setu an affidavit is to be signed by the .executive magistrate, Tahsil office, Amravati. I am confused whether the beginning is as "In the court of Ho'ble Executive magistrate or Before the Ho'ble Executive magistrate,Amravati" which format is correct. Please guide me accordingly.Thanks
