Are you salaried employee? If so, you must be declared your tax deduction details with your HR. 

I think time has passed. Form 12bb used to submitted on January every year. 

If you miss to declare your tax deduction, don't worry, there is a provision. you can claim your deduction. But, all your tax deduction proof to be in handy.

AO may issue an intimation that you claimed more than your employer declared in form 16A.

You should give a declaration to close the assessment. And get refund if there is any excess amount.
