If receipt in year are less than ,50 lacs you can opt 44ADA presumptive income scheme and file ITR 4

Since Capital gains is there - file ITR3

Yes ITR 3 due to capital gains ,
the nature of receipt is salary... you can go for ITR 2 also

YES MISSED THE FORM 16 PART.   It is form ITR-2 only.

Is this the same doctor having these incomes also or a different assessee?

A doctor work in hospital and receive form 16 and he also have short term and long term capital gain from equity on which security transaction tax is paid.

he also have speculative income which is loss of ₹300.
so which ITR is to file and how to show the speculative income(loss).

He can File ITR-2  - salary + Capital gains (LTCG + STCG)
