You can get it on the website of the institute. They have provided it only today.

i am impressed with institute online exam application :-) it took max 15 minutes, earlier it took almost a day or two to do such excercise.. (prepare demand draft, now online payment option is there)
I did my online payment and took a print out of it & the payment proof...Now is the procedure over or should i sent a copy of the same to kolkata?
@ Narayanan, if you paid fee thru online, then no need to send printed appln form, its reqd only if fee paid thru DD..,
