What is my 4-digit pin(personal identification number)?

PIN in Exam form is ur Person Identification no. which can be use in future reference with ICAI.

it is choosen by you like your Password.... choose any 4 digit and keep remeber that.

i'm not filing my form online, but still being asked for my PIN no...

should i create it now? or should i use the one i filled for cpt registration?

I think you'll you have to make a new one. I didn't have a PIN number in CPT. 

i forgot my pin number what should i do?

I case you have forgotten the PIN, you should login again by forgetting the password and reset the account which will then give you an option to generate the PIN and other credentials again on the portal.
