in Row 13 of 8.Details of Cenvat Crdit taken and utilised.

"You have to show 10% of value of the exempted goods/8% of value of exemped services as per rule 6 of cenvat credit rules ammended."

in Row 14;

You have to show the other payments if any? other than the  above mentioned?
