What is "Proof of withdrawal with Number & Forum" to be attached along with form-4 under Vivad Se Vishwas.

I think it's the appeal number which was generated at the time of filing of appeal with the CIT for your case under dispute. And "Forum" looks to be a type. I think you meant "Form"
