Vat audit in west ebngal

When is the last date for submitting form 88 in west bengal...within 31.12.2012

where is it required to be  submitted.....To respective charge officer

Also Can any1 plz provide me with A copy of Form 88 (File Attached - Old Format)
Note: Old format for the year 2011-12 will be accepted. From the FY 2012-13 onwards shall have to submit in the new format. (Trade Circular No. 17/2012, Dated:  09.11.2012)
