Your Name in PAN is Different That of in Add. Proof.

I Suggest to use other Proof of Identity and Address from any of the follow-

i have 2 id proof:pan card and voter id

                                VOTER ID

Name: Rathod Rajubhai

Father's Name: Gordhanbhai

                                 PAN CARD

Name:Rathod Rajubhai G

Father name:Gordhanbhai Karamshibhai Rathod

                                TELEPHONE BILL

NAME:Rathod Rajubhai Gordhanbhai

                       Now which id proof can i use for din? How can i fill first,last,and middle name in apllicant name and How can i fill Father's Name (first,last and middle name) In Din apllication?
