Just send your Pan card  for ID Proof and Telephone Bill for address proof after due attestation. Telephone bill must not be older than 2 months.

Fill father's name as it is

1st =Gordhanbhai

middle = Karamshibhai

last = Rathod

And How Can I fill My name as first last and middle name?

Dear Raj,

You must fill all the information matching with the proof u r attaching with the form. DIN is rejected because smtime the information given in form does not match the proof attached.

If u r giving Passport as proof then fill according to the information given in passport, even if there is some variation.

Jst give voter id as proof and not pancard and fill details according to voter id. Dont send PAN CARD.

But my voter id have single name.see above given pan cards name and say how can i fill my first,middle and last name?

Yes you should send your PAN Card..

and mention name as under:-

This is also just to inform you that after obtaining DIN you can easily change the details but the need is valid supporting thereof.

First name: Rathod

Last name: Rajubhai

Fathers first name: Gordhanbhai

i attech here my din form which is rejected. pls see and say me what change need in there and which document can i send ,it give above.

First of all, dont mention your PAN CARD no. as it is not mandatory and dont attach it with the form. Voter id will do the work of both, ie. id proof and address proof. So attach only voter id.

As per my last msg. fill the form as it is mentioned in the Voter id . In voter id , generally, first name come first and last name come in the end. Bt in your case ur last name Rathod is coming first so you have to fill Rathod as first name and rajubhai as last name. Ur father's name contain only one word which will cm in first name.

Fill the details as it is mentioned in voter id.
