Urgent : claiming exemption

Hello Friends,

Below is my Group III Dec'11 result.I want to take exemption in P12 and
re-appear in remaining 3 papers.Kindly let me know the procedure of
doing the same.
              P-11    47,  P-12  60  , P-13  46 ,  P-14   43  

also let me know the amount of DD to be made.do we need to pay any fees for claiming exemption?
