Two business with same gstn

One of my client having  different business in same state, one is vehicle renting to industries while other is stationery business, local work contract of construction etc. . he has obtain the GST No for work contract now he wants to sell stationery also to Registered person, so whether he need to take separate ragistration or can give invoice with same GST No. all business are in same state but with different names.

all businesses with different trade name requires separate Registrations for each business.
In case of same trade name but different business, the said business/ products/services shall be registered while registering

But if that assessee is decided to club all such business with registered name with gstn. can he do the same means use single gstn and if now he wants to do so wheteher he need to obtain new regitration ? at present in his registration nature of business its mention as work contract and others. 

If has to modify which form to be filled and requied any additional documents ?

I guess you are trying to say that at present the assessee has obtained registration by registering the works contract services and other services. Right? So in this case the stationery business is not a service industry, hence for registering other services and goods you just have to go on GST portal, login to assessee's account, select amend core fields, and you can change or add the type of goods or services you want to add to your existing Registration. The benefit of doing this is that you can amend your existing Registration and you don't need to get a fresh registration for this and there will be no need to get a new GSTIN number.
