Transporting furniture purchased in kerala to karnataka

Hi All

I have purchased furniture in kerala which needs to be transported to my house in Bangalore, Karnataka. The furniture is only for personal use and not for selling.

The furniture manufacturer has informed me that a letter/form is to be taken from the Sales tax dept/ municipal office stating that the furniture is for personal use only and given to him so that it can be presented at the karnataka checkpoint.

Request help on what is the form and who is the issuing authority? Is this really required or will a letter stating the same and my ID/Address proof suffice?
