Training completed with excess leave

Hi guys..

Situation:

I started my training from 12.07.2008, then took transfer on 23.04.2011 and got registered from 15.05.2011 under new CA. Thats why date of completion got extended to 1.3.2012.

Question:

My training completes on 1.3.2012

I took excess leaves of 22 days

i want to continue with the employer with whom my training is getting completed on 1.3.2012

Which form is to be filled up apart from 108 for this excess leave and what date to write in date of commencement of secondary articleship? 1.03.2012 or 2.03.2012?? PLS HELP........

I've done my best to keep my question clear..... Pls answer ans Bhagwan apka bhala kare... pls..
