If you've lost your login ID and password for filing tax returns, and can't receive OTPs due to the consultant's demise, there are several recovery options. Your login ID is your PAN. You can attempt a password reset via net banking, which allows you to update your registered email and mobile details afterwards. Alternatively, contact the helpline for assistance; they can send a reset link to a new email address after verification. Checking your last intimation under section 143(1) might reveal the registered mobile and email. Linking your Aadhaar to the IT portal also offers an OTP-based login and password reset method.
