A survey is being conducted to collect opinion and valuable suggestions from various stakeholders. Members, students and others are requested to fill in and submit the form. The last date to submit the form is 31st January, 2013.

hi,

it looks like a form which collects the personal information. first thing is if icai need to take survey it will take in their website only why they have to go to google docs. this doesnt look genuine.
