in exm form.... not select the option of exmptn.. also not fill the rqurmnt of this option
fill the form as if you are appearing for that group first time...and voila your exemption will be forfeited !!!!

The correct Procedure is you have to Send a Letter in Your Own Handwriting to ICAI to this effect & if I am not mistaking then you have to probably give an Affidavit to this effect that you wish to surrender the exemption. Its better you put your query on e sahayataa & you will get the proper & detailed latest provisions for your query.
uma please reconsider before surrender exemption.
