St - 2 amendment

As we want to make amendments in ST 2 certificate of my auditor can u tell me what and all documents we should attach while making changes for the following 1.Name 2.Address 3.Service Tax Registration No - there is a alphabetical mistake in Service Tax Registration No.. from when service tax to be paid on bill basis if there is any notification pls do attach
